At Jordan’s Home Health Solutions Inc., we are dedicated to providing compassionate, personalized care to individuals in the comfort of their own homes. Our founder, Jordan, was inspired to start the company after witnessing her father’s struggle with a stroke 20 years ago. As a young child, she took on the role of caregiver, learning firsthand the challenges of supporting a loved one with health needs.
This experience shaped Jordan’s passion for improving the lives of others facing similar challenges. She understands the emotional and physical toll that caregiving can take, and it is this empathy that drives our commitment to offering the highest quality of care. Our team is devoted to making sure every patient receives the attention and respect they deserve, empowering them to live with dignity and comfort at home.
We Offer Exceptional Home Care Services
Locally owned and operated, Jordan’s Home Care is your extended family when your family is not able to be there. We focus on providing competent, quality care to all of our clients and aim to keep your loved one where they are most happiest: at home!
1. Initial Call:
After receiving your information, we will conduct a 5-10 minute call to answer any questions and set up a time to conduct an assessment.
2. In Home Assessment:
During the assessment, we will ask questions that will assist us with your individualized care plan, answer any additional questions, and complete paperwork.
3. Start of Care:
At the start of care, a supervisor will come with the Caregiver and present the care plan and assist with introductions.
4. Client Visits:
Once care has begun, we will continue to ensure you are receiving exceptional care and adjust our care plan if needed with quarterly nurse visits
